San Jacinto River Bottom Outreach Team Creating Hope Through Housing

There is no better feeling than helping clients stabilize and begin a path toward long term success. Walking alongside individuals as they share the traumatic experiences that led to their homelessness, while providing ongoing support through their journey. Together, we navigate each step, overcome numerous hurdles, and help people rebuild the foundation they need to become self-sufficient and thrive again.
The San Jacinto River Bottom outreach team has served 530 individuals and successfully helped housed 316 individuals, while providing street outreach services and comprehensive wrap around care throughout the Hemet and San Jacinto region. Through this work, clients receive housing assistance, consistent street outreach, mental health and substance use support, and workforce connection programs designed to promote long term stability.
Our Street Outreach Teams meet people where they are, building trust through in‑person engagement and opening pathways to critical services. Housing and shelter programs offer safe spaces for those ready to transition off the streets, while comprehensive care including behavioral health support, substance‑use counseling, and employment programs made possible through strong partnerships.
